What To Expect From A Photofacial
A photofacial or IPL, is a 30-minute procedure in which intense pulses of light are used to penetrate deep into the skin. Photofacial then causes collagen and blood vessels below the epidermis to constrict, reducing redness and age lines. The procedure involves only minimal discomfort, while the redness and swelling that sometimes occur after treatment disappear shortly. Most benefits of a photofacial occur gradually in the weeks following treatment
DAY OF THE PROCEDURE
Before the procedure begins, make sure your skin is free of any tanning products or makeup. Avoid prolong sun exposure and any topical retinoids. Your skin will be cleansed and a cool gel will be applied to the treatment area, both to lubricate the handpiece and cool the skin. You will be given eye protection to protect your eyes from the bright flashes of light.
DURING THE PROCEDURE
During the IPL/photofacial procedure, you will feel a warm sensation caused by the handpiece. Some patients describe the sensation to a light “snap” from a rubber band. The number of pulses you receive will depend on the size of the area being treated. Following the procedure, your skin care specialist may help you in icing down the treatment area to help soothe any discomfort from the procedure. This is often unnecessary, however, as IPL/photofacial treatment causes very little pain.
AFTER THE PROCEDURE
The recovery time from an IPL skin procedure is often very short, as there is very little damage done to the skin during treatment. Depending on the skin condition you are seeking to remedy as well as your skin type, the redness that is present following the procedure may persist for a longer or shorter period of time. The side effects of IPL/photofacial treatment dissipate within a few hours for many patients. For some patients, however, the redness or dark spots may take a week to fade. Patients treated for severe acne sometimes experience longer recovery times, though they are often amazed by the results when the side effects wear off.

From topical treatments that lighten to in-office lasers that ablate the upper lay of skin, there are a myriad of options to limit the look of the sun-damaged skin.  Topicals are almost always the first recommendation to address sun damage, and are often coupled with more aggressive treatments.
Topical Treatments:
Start with retinoids- and retinol, which are derivatives of vitamin A.  They work to thicken collagen, regulate cell turnover and lighten spots.  Add in serums with growth factors and peptides to strengthen skin that has been weakened by the sun. Lighten up with skin brighteners that diminish pigmentation in the skin.  Hyrdoquinone is the most effective bleaching ingredient, but licorice soy and kojic acid help, too.
If you have freckles and minimal spots: IPL (Intense Pulsed Light)
Although the least intense type of sun damage- they can be dark and large in quantity. Freckles tend to surface on the skin as early as childhood.  IPL treatments are the best tool to get rid of both red and brown pigmentation.  It does a great job of normalizing color, so you can really get those light and dark brown spots.  The more you do this, the more collagen that forms, so your skin looks healthier and smoother.  IPL is a great treatment for someone who isn’t ready for a factional laser yet.
If you have severe age spots: Hydroquinone, Kojic Acid, Retin-A, and Laser Resurfacing
Ranging in color from brown to almost black and in size from something bigger than a freckle to as big as a coin, age spots are a direct result of excessive sun exposure that has prompted an overproduction of melanin.  When it’s an isolated brown spot that isn’t too dark, hydroquinone or kojic acid, used with Retin-A, can help reduce discoloration.  At The Paragon Plastic Surgery & Med Spa, we have our Paragon Cream that offers all 3 ingredients. More aggressive procedures are offered, like lasers that really work to resurface the skin.  The principle is that the body recognizes a wound (created by the laser) and heals it with the new collagen and new skin.
If you have skin that is uneven in texture: Exfoliation and Chemical Peels
The sun sucks the life right out of your skin.  After skin is damaged, it looks older than it should and more leathery in texture.  It’s not smooth or youthful looking because there is a lack of healthy collagen and elastin, making the skin look rough, dull and weathered.  Regular exfoliation with salicylic, lactic and glycolic acids can help.  You’ll see the most improvement when they are used in tandem with a prescription strength retinoid to further smooth the skin.  Chemical peels physically exfoliate the top layer of skin and repair some of the damage with specific ingredients, namely acids.
If you have red broken blood vessels: IPL Photofacials
Small and thin, blood vessels can easily break from anything from stress, pregnancy, being too aggressive on skin and even the sun.  But the problem comes when skin tries to repair them and can’t.  Cumulative sun damage prevents skin from regenerating these damaged vessels properly and so the redness lingers.  Broken vessels respond well to IPL.  The laser works to take the redness out because the wavelength of the machine targets the hemoglobin in blood, which is what gives the skin the redness.  The vessel almost bursts or is reabsorbed by the body over time and the broken capillary goes away.

Cosmetic Procedures- Laser Skin Rejuvenation
Today’s lasers can diminish many signs of aging — from early wrinkles and age spots to deep lines and spider veins. A laser procedure often can be performed in less than an hour and may require little or no downtime.
Also Called
Numerous lasers and light devices are used to rejuvenate the skin. These devices are generally grouped into the following categories:

  • Non-ablative laser rejuvenation – No downtime. Best for early signs of aging.
  • Intense pulsed light (IPL) – Not a laser, this light therapy penetrates deeper into the skin than a non-ablative laser. IPL does not injure the surface of the skin, so there is usually no downtime. IPL can improve skin texture and color as well as reduce pore size.
  • Photodynamic therapy (PDT) – This is a two-step procedure. First, a solution that makes the skin more sensitive to light is applied and left on for 30 minutes or longer. Then light from the laser or other source is beamed onto the treated area. Downtime is minimal. PDT decreases redness, reduces age spots and freckles, and improves the overall texture and appearance of the skin.
  • Fractional laser rejuvenation – Offers deeper rejuvenation. Recovery time is short. Best for diminishing signs of aging such as age spots, wrinkles, and skin discoloration.
  • Ablative laser resurfacing – Heats the skin to cause a visible wound, which means some downtime for recovery. Effective for deeper wrinkles and diminishing aging caused by years of unprotected sun exposure. A type of laser called a fractional ablative laser causes no visible wounds. Fractional ablative laser resurfacing requires less downtime than ablative laser resurfacing.

Signs of Aging Treated
Lasers and other light-based devices are used to treat these concerns:

  • Age (sun) spots
  • Dark circles under the eye
  • Dry, dull skin
  • Fine lines
  • Freckles
  • Hand rejuvenation (age spots, loose skin, and lines)
  • Melasma
  • Redness and flushing
  • Scars
  • Seborrheic keratoses (flat only)
  • Skin texture (treats roughness)
  • Skin tone (treats mottled complexion and skin discoloration)
  • Spider veins
  • Stretch marks
  • Tattoos (removal)
  • Unwanted hair
  • Varicose veins
  • Wrinkles

When Do You See Results from Laser Skin Rejuvenation
Improvement is gradual. It may take several months to see the full effects. When a laser that does not require downtime is used, a series of treatments may be necessary to see the desired results. The number of treatments varies from person to person; however, between 3 and 6 treatments is the norm.
Effectiveness
Research shows that laser rejuvenation can dramatically reduce fine lines around the eyes and mouth and on the cheeks. With 1 treatment, age spots can be diminished by 50% and may be completely eliminated with follow-up treatments. Laser skin resurfacing can significantly reduce deep lines, effectively remove some shallow scars, and tighten the skin.
While lasers may sound like magic wands, they are not. It is important to have realistic expectations. Laser skin rejuvenation can be effective, but does not work for everyone.
Some skin tightening is possible, but lasers cannot produce the same degree of lifting seen after a surgical facelift or eyelid lift. Wrinkles caused by constant muscle movement such as frown lines can be hard to treat with lasers alone. Another procedure such as botulinum rejuvenation may be used along with a laser to obtain the desired results.
How Long Results Last
It is possible to permanently remove some signs of aging such as a spider vein, age spot, or scar. However, laser skin rejuvenation cannot stop the skin from continuing to age. Crow’s feet may re-appear and new age spots can develop. Because sun exposure accelerates signs of aging, protecting the skin from sun exposure will lengthen the amount of time that the results last. Protecting the skin from the sun also can prevent some signs of aging from re-appearing such as blotchy skin and dark patches.

A LINE
Small and fine, a line tends to occur from repeated muscle movement that etches a slight mark into the skin.  Those who smile, frown or squint a lot develop fine lines earlier than those who don’t make these facial expressions as often.  When skin is young it snaps back into place, but as we get older it loses elasticity, so that ability to spring back becomes harder and less frequent, causing more premature lines.
A WRINKLE
Thicker and more dramatic in nature, wrinkles are noticeable folds, ridges and creases in skin.  Wrinkles can be caused by age, collagen and elastin breakdown, too much sun exposure, medications and dry, dehydrated skin.  Wrinkles are different from lines in that they are more diffuse throughout the face.
When a Line turns into a Wrinke
A line can turn into a wrinkle, especially when left untreated.  A fine line occasionally deepens over time, turning into a wrinkle due to further breakdown of collagen.  Just like how a line can develop into a wrinkle, a wrinkle can reverse into a line when it’s reduced with the right treatment, making it less apparent.
