Breasts

Whether you want to increase your breast size, get a lift, go down a size or have breast revision surgery, the office of Dr. Mark Bishara has the information for you to get started on your smart beauty choice.

Breast Augmentation

Also known as augmentation mammaplasty, breast enlargement or breast implants
Breast augmentation surgery increases or restores breast size using silicone gel implants, saline implants or in some cases, fat transfer. One of the most popular and frequently performed aesthetic surgery procedures, breast augmentation has a long and successful track record in satisfying women who wish to enhance, regain or restore balance to their figures.

When to Consider Breast Augmentation

  • If you want a better proportioned or more appealing figure
  • If you wish your clothes fit better
  • When pregnancy, weight loss or aging has affected the size and shape of your breasts
  • To restore symmetry if one of your breasts is smaller than the other
Also known as mastopexy and breast lift surgery
A breast lift addresses sagging and uneven breasts, decreased breast volume and drooping nipples and stretched areolas (the darker area surrounding the nipples), recreating a youthful shape and lift to your breasts. If there is too little or too much breast volume, a breast augmentation or breast reduction might be recommended in addition to a lift. Every year, thousands of women undergo successful breast-lift surgery, experience no major problems and are pleased with the results.

When to Consider a Breast Lift

  • Pregnancy, nursing, gravity, weight gain or loss, normal aging, and heredity have taken a toll on the shape of your breasts, resulting in sagging
  • If your surgeon thinks that breast implants alone are unlikely to achieve the contour you desire
  • If the tissue surrounding the nipple or areola has become stretched

Breast Reduction

Also known as reduction mammaplasty
The goal of breast reduction surgery is to reduce the size of your breasts and reshape them so that they are proportionate to the rest of your body and are no longer a source of physical discomfort. This commonly requested, predictable procedure has the dual benefits of improving your appearance while relieving the physical and emotional burden of overly large breasts.

When to Consider Breast Reduction

  • If your breasts are too large for your body frame and create neck, back, or shoulder pain
  • If you have heavy breasts with nipples and areolas (pigmented skin surrounding the nipples) that point downward
  • If one breast is much larger than the other
  • If you are unhappy and self-conscious about the large appearance of your breasts

Breast Revision

Also known as breast implant replacement
Breast implant revision surgery, which commonly involves removal and/or replacement of saline or silicone breast implants, is performed to change the size or type of your implants and to correct any complications from your primary breast augmentation. The overall goal is to restore a youthful breast contour and appearance.

When to Consider Breast Revision

  • Your saline breast implants have deflated or an X-ray or MRI suggests your silicone implant has a shell leak
  • If you want to change your implant/breast size
  • If you have tightening of the scar tissue around the implant (capsular contracture) or your breast implants have shifted in position
  • If your breast tissue has changed as a result of skin stretching or weight loss/gain

This New Year’s, why not do something for yourself that you know will have you feeling great for the next 365 days? Many women will be making the decision to undergo breast enhancement surgery to start 2014 with a boost in self-confidence and a positive self-image.
Most Women are Unhappy With their Current Breasts
It’s a fact. Whether they’re too small, have lost shape or volume after pregnancy or weight loss, are asymmetrical or even too big. This can often lead to a negative self-image and a lack of confidence in both one’s personal and professional life. Breast augmentation continues to be the most popular cosmetic surgery in the United States since 2006 because many women – around 300,000 a year – are doing something about it.

Breast enhancement procedures like augmentation have also been tied to an increase in self-confidence both in a woman’s personal and professional life. Why? When someone feels good about the way they look, it often translates to being more comfortable dealing with the everyday aspects of life.
Isn’t that what New Year’s resolutions are all about? Doing something to make sure you feel better during the next 365 days than you did the last?
Breast enhancement surgery has helped many women feel more confident and happy with their everyday lives. The New Year is the perfect time to start with your ideal breasts, because you have enough time to heal and look great for Valentines as well as the upcoming bikini season.
Ninety-eight percent of women undergoing breast augmentation surgery say the results met or exceeded their expectations, according to a prospective outcome study published in the May issue of Plastic and Reconstructive Surgery®, the official medical journal of the American Society of Plastic Surgeons (ASPS). Women also report improvements in self-esteem and quality of life after breast augmentation. The study adds high-quality information regarding expected outcomes after breast augmentation – including recovery time and psychological benefits.
On a 10-point scale, the women reported an average pain score during recovery of 5.9. On average, they used prescription pain medication for five days and were off work seven days. Patients felt they were “back to normal” about 25 days after surgery.
Eighty-five percent of women rated their new breast size “just right.” Thirteen percent would have preferred a larger size and less than two percent a smaller size. Only one percent expressed dissatisfaction with their scars, which were usually located in the crease under the breast. Seventy-five percent of women rated their breast firmness “just right.” When asked to rate the result, over half of the patients gave it a perfect 10 on a scale of 1 to 10. Almost all patients (98 percent) reported that the results met or exceeded their expectations.
Breast augmentation is the most popular cosmetic surgery procedure in the United States – approximately 286,000 operations were performed in 2012, according to ASPS statistics. The study results provide new information for women considering breast augmentation, particularly in terms of their expectations of the recovery and results. “This study is valuable in providing information from the patient’s perspective, not the surgeon’s,” according to Dr. Swanson. “Patient satisfaction is the most important measure of surgical success.”

Women also report improvements in self-esteem and quality of life after breast augmentation, according to the study by ASPS Member Surgeon Dr. Eric Swanson, a plastic surgeon in private practice in Leawood, Kan. The study adds high-quality information regarding expected outcomes after breast augmentation – including recovery time and psychological benefits.
98 Percent of Women Satisfied with Breast Augmentation Results
The survey study evaluated 225 consecutive women who returned for interviews at least one month after breast augmentation over a five-year period. Interviews included questions about the recovery, results, complications and psychological effects.
The average patient was 34 years old, and the majority of women received saline-filled breast implants placed under the muscle. The average implant volume was 390 cc.
On a 10-point scale, the women reported an average pain score during recovery of 5.9. On average, they used prescription pain medication for five days and were off work seven days. Patients felt they were “back to normal” about 25 days after surgery.

Almost all patients (98 percent) reported that the results met or exceeded their expectations.
Increased Self-Esteem and Other Psychological Benefits
Nearly 40 percent of the women surveyed experienced at least temporary nipple numbness after surgery. Persistent numbness was reported by only two percent. The complication rate reported by patients was 10 percent. When asked about psychological effects, 92 percent of women reported improved self-esteem after breast augmentation and 64 percent reported an improved quality of life. Before surgery, the majority of women (86 percent) were self-conscious about their breasts. After surgery, only 13 percent were self-conscious about them.

Before and After Pictures of Dr. Mark Bishara’s Patients:
Are you a good candidate for breast augmentation?
One or more of the following feelings or conditions may indicate that you are a good candidate for breast augmentation:

  • you are bothered by the feeling that your breasts are too small
  • clothes that fit well around your hips are often too large at the bustline
  • you feel self-conscious wearing a swimsuit or form-fitting top
  • your breasts have become smaller and lost firmness after having children
  • weight loss has changed the size and shape of your breasts
  • one of your breasts is noticeably smaller than the otherDuring the consultation, you will be asked about your desired breast size and anything else related to the appearance of your breasts that you feel is important. This will help your surgeon to understand your expectations and determine whether they realistically can be achieved. How will my plastic surgeon evaluate me for breast augmentation surgery?Your plastic surgeon will examine your breasts and take photographs for your medical record. He or she will consider such factors as the size and shape of your breasts, the quality of your skin and the placement of your nipples and areolas (the pigmented skin surrounding the nipples). If your breasts are sagging, a breast lift may be recommended in conjunction with augmentation.
You should come to the consultation prepared to discuss your medical history. This will include information about any medical conditions, drug allergies, medical treatments you have received, previous surgeries including breast biopsies, and medications that you currently take. You will be asked whether you have a family history of breast cancer and about results of any mammograms. It is important for you to provide complete information.
If you are planning to lose a significant amount of weight, be sure to tell your plastic surgeon. He or she may recommend that you stabilize your weight prior to undergoing surgery. If you think that you may want to become pregnant in the future, you should mention this to your surgeon. Pregnancy can alter breast size in an unpredictable way and could affect the long-term results of your breast augmentation. There is no evidence that breast implants will affect pregnancy or your ability to breast-feed, but if you have questions about these matters, you should ask your plastic surgeon.

What type of breast implants can be used for breast augmentation?

The implant is placed in a pocket either directly behind the breast tissue (right) or underneath the pectoral muscle which is located between the breast tissue and chest wall.

The size and type of breast implant recommended for you will be determined by your goals for breast enhancement, your existing body frame, and mass, your existing breast tissue, and the preferences you and your plastic surgeon discuss.  All breast implants include a solid silicone rubber outer shell, called a lumen.
    Breast implant options include:

    • Saline breast implants: Filled with sterile salt water
    • Silicone filled breast implants: Filled with soft, elastic gel

New scientific data on the safety of breast implants is rapidly being collected. In the future, it is possible that additional types of filler materials may become available as advancements take place. Surgical techniques for breast augmentation and breast implants themselves are continuously being refined, increasing the safety and reliability of the procedure. Your plastic surgeon will be able to provide you with the latest information.
