Ultherapy
Do you need a lift? Are you looking in to doing something for the loose skin on your neck or jowls but are not ready to go under the knife? Ultherapy might be the treatment for you! Ultherapy is the only non-invasive treatment that is FDA-cleared to lift the skin on the neck and under the chin and the eyebrow. It is also newly approved to treat the lines and wrinkles on the décolletage. Here at Paragon Plastic Surgery and Med Spa, Dr. Mark A. Bishara and his staff along with Ultherapy can give you a more youthful, natural looking you!
How does it work?
Ultherapy uses safe, time-tested ultrasound energy to stimulate collagen production. The energy is specifically delivered to the same layer treated during a face lift without cutting or harming the surface of the skin. The ultrasound energy heats the layer between 60-70 degrees centigrade that triggers Neo-collagenesis.
What to expect?
Depending on the area being treated, the treatment may take 60-90 minutes to perform. Your skin will be cleansed and marked appropriately for the area to be treated. As the ultrasound energy is delivered you may feel some mild to moderate discomfort. It is recommended to take some over the counter pain relievers before your treatment and comfort levels vary from patient to patient but the sensation only lasts while the energy is being delivered. You may have some slight redness and swelling that dissipates within a few hours. Some patients may experience  tenderness and bruising that may last a few days.
How many treatments do I need?
Most patients only need one treatment. However, depending on the degree of laxity and collagen-building process, some may benefit from additional treatments. Since the aging process never stops annual touch up treatments can prolong their results.
Is there any downtime?
There is no downtime with Ultherapy. You are able to resume all your normal activities and skin care regimen at home.
When will I see results?
You may see some initial effects from the treatment right away but generally the ultimate results will take place over 2-3 months and up to 6-9 months.

We all know that botox works for wrinkles but did you know it can work for breasts? Botulinum toxin type A can now play a role in inhibiting one of the most serious breast surgery complications-  capsular contracture.
Capsular contracture is an immune system response to foreign materials in the body. It mostly occurs from complications from breast implants and artificial joint prosthetics. It is followed by the formation of capsules of tightly woven collagen fibers tightening and compressing to the implant. It can occur in different grades and become very discomforting and painful to the touch.
Now researchers have found that botox can reduce the capsule thickness by injecting 0.5 mL into implant pockets and found it had also decreased inflammatory cells. Once validated in a clinical setting, this may be another use for the neurotoxin. In a published article in 2015 in the Archives of Plastic Surgery authors wrote about botox’s role and suggesting it may be effective in reducing keloid scarring, which is similar to implant capsules.
Current uses for Botox include treating facial wrinkles, hyperhydrosis, blepharospasm and migraines. This could be just another beneficial use for the neurotoxin after performing more patient research.

The year is flying by. One year older, Yikes! The worst part is that not only do we add another number to our age but our appearance begins to change as well. Wrinkles start to show more and more. As if wrinkles weren’t bad enough our skin begins to droop as well. Sagging neck is a common one.
So what exactly causes a sagging neck?  At the Paragon Plastic Surgery and Medspa we know that loose skin is a part of aging but can also be genetics along with weight.  Dr Bishara says, “Excessive sun exposure can have a lot to do with it as well. ”  Yes, those uv rays can be very harmful!

What you can do to fix it!

At The Paragon Plastic Surgery and Med Spa we offer surgical and non-surgical treatments to help with aging skin. Facelifts have become quite popular with Dr. Mark Bishara as no one likes to look older nor have loose skin. A facelift counteracts the effects of aging, gravity, sun exposure and stress by removing fat and tightening skin and muscles, giving your face a fresher, more youthful look and reducing the appearance of wrinkles, creases and fat deposits. Many patients look 10 to 15 years younger after a facelift.
If surgery is not what your looking then a NIR Skin Tightening treatment can help. Near infrared technology is used to penetrate deep heat to the deepest layers of tissue. This stimulates collagen production and tightens lax skin. Nearly any patient seeking fuller, tighter skin can benefit from laser skin tightening as a safe, noninvasive and effective procedure. The laser skin tightening procedure can be fully customized to help each patient receive the best possible results, depending on the skin condition, size and location of the targeted area and the strength of the laser exposure.

What is a Facelift?

A facelift purpose is to remove excess skin and tighten the underlying tissues and muscles on the face and neck. Facelifts are also used to correct any sagging in the face and those annoying hard to get rid off double chins. If you are experiencing any of the above then Dr. Mark Bishara at The Paragon Plastic Surgery and Med Spa is the surgeon you want to see!

What are facelift steps?

Ok so you’ve met with Dr. Mark Bishara and now its time to get things done. The incisions that will be made depend on the area that needs the change. After the incisions have been made there will be various degrees of “undermining” performed on the skin as the deep layers of your skin get lifted. What is undermining? Undermining is the separation of the overlying skin of the face and neck to the muscles and deep tissues of the skin. Soon after your skin will be raised from the temples, cheeks and neck and lifted and reposition. Any excess skin and fat will then be removed. The skin will then redrape over the new underlying structure and close with stitches.  You will then be wrapped with padded dressing.

How long will it take to recover?

Walking and light activities can be resumed 3 to 7 days after surgery. Avoid anything that puts pressure on your face such as lifting, bending, and straining for 7 to 14 days.  Try to suppress any forceful coughing or sneezing for 2 weeks. Avoid aerobic or heavy exercise for at least 3 weeks following surgery. Some swelling and bruising will develop and is minimized by resting in an elevated position. Rest and sleep in a reclining position with your head elevated 45 degrees for at least 2 weeks after surgery. You can resume make up after stitches have been removed. Scarring can be virtually invisible behind the ear, as incisions are usually made in hidden areas where the scars will look like small wrinkles.

We have Dieter Manstein, MD and R. Rox Anderson, MD to thank for a study done decades ago who observed children that consumed a lot of popsicles were freezing and removing fat cells within their cheeks creating dimples. Today that same technology has been recreated by Zeltiq the makers of Coolsculpting. We’ve all heard the name, whether on TV, radio or on magazines. But what exactly is Coolsculpting?
Cool sculpting is the ONLY FDA cleared, non invasive, fat eliminating procedure. This means no cutting, no needles, and no down time. The best part, it removes fat cells permanently.
So how does this work and am I candidate? Let’s start who this is not for…
This is not for someone who is looking at this as removing 5 lbs of fat in one session. Or a cure for poor dieting or lack of exercise, rather this is for people who can’t get rid of that stubborn belly fat that diet and exercise fail to reduce. The love handles that crunches won’t cure and the annoying fat on the back of the arms a million push ups just won’t fix. You and your consultant will discuss areas of concern, about the treatment, cost, and number of treatments needed for your desired results. Practitioners note that most patients have 20%-25% percent reduction of fat cells in the treated area after ONE treatment.
Areas of interest from potential patients include the chin, upper and lower abdomen, flanks (sides), love handles, inner and outer thighs and thanks to popsicles in that earlier study we know that fat cells have been proven to not survive extreme cold temperatures.
Treatment time is approximately one hour, you will rest comfortably while a technician drapes a cool membrane barrier against the treatment area. This protects the skin from the coldness. The device is placed over the membrane and the device will pull the area of concern in by a vacuum mechanism. This should pose little discomfort and numbing to be in full effect after approximately 10 mins. Once the device is pulled away from the treated area the skin will have taken on a form, almost like a stick of butter. This is what you should expect! The technician will then massage the area until flat again.
Recovery after CoolSculpting is truly a no downtime process. The treated area will be numb and you may experience a little swelling or discomfort, nothing an Ibuprofen cant handle. However you may resume your day to day activities immediately.
So what happens to those fat cells? Where do they go? The expired fat cells will naturally be metabolized and expelled from the body. Result are seen as in little as 3 weeks, but best by month 2 and 3.
How much is this going to cost me? The price for CoolSculpting varies on a few factors, areas of concern, how many treatments you may need and what is your end goal. With that said, most treatments start at $600 a session.
What is the difference between fat reduction and weight loss?
Fat reduction is permanently reducing the number of fat cells within a given area. Those cells will never come back. Weight loss allows the fat cells to reduce in size however, does not eliminate them permanently from the body.
The traditional method is Liposuction and is a preferred method for those seeking larger amounts of reduction in one session. However, will be performed by a surgeon, under anesthesia with some recovery time. CoolSculpting offers us the same technology for treating a smaller area over one or several sessions. Speaking to your plastic surgeon is the best way to find out which procedure might be better suited for you.
