#5 Plastic Surgery is All About Beauty and Vanity

Plastic surgeons care about the functional and overall results after surgery, not just the aesthetics (although we are specifically trained to optimize those, as well). When a patient does choose to have surgery for cosmetic reasons, it is often to correct areas which are not amenable to diet, weight loss or nonsurgical procedures.
#4 Plastic Surgery Patients are the Rich and Famous
If plastic surgeons only operated on the 1%, we wouldn’t have a thriving plastic surgery industry.
Most aesthetic patients are not the rich and famous, but are average people who simply wish to restore confidence, enhance their overall appearance and improve their lives.
#3 Only Women Get Plastic Surgery
Although women have been the traditional customers of plastic surgery, men are turning to plastic surgery in increasing numbers.

#1 Plastic Surgery is the Same as Cosmetic Surgery
All plastic surgeons do not have the same training. Yes, they are board certified – but not in plastic surgery.

Ninety-eight percent of women undergoing breast augmentation surgery say the results met or exceeded their expectations, according to a prospective outcome study published in the May issue of Plastic and Reconstructive Surgery®, the official medical journal of the American Society of Plastic Surgeons (ASPS).
Women also report improvements in self-esteem and quality of life after breast augmentation, according to the study by ASPS Member Surgeon Dr. Eric Swanson, a plastic surgeon in private practice in Leawood, Kan. The study adds high-quality information regarding expected outcomes after breast augmentation – including recovery time and psychological benefits.
98 Percent of Women Satisfied with Breast Augmentation Results
The survey study evaluated 225 consecutive women who returned for interviews at least one month after breast augmentation over a five-year period. Interviews included questions about the recovery, results, complications and psychological effects.
The average patient was 34 years old, and the majority of women received saline-filled breast implants placed under the muscle. The average implant volume was 390 cc.
On a 10-point scale, the women reported an average pain score during recovery of 5.9. On average, they used prescription pain medication for five days and were off work seven days. Patients felt they were “back to normal” about 25 days after surgery.
Eighty-five percent of women rated their new breast size “just right.” Thirteen percent would have preferred a larger size and less than two percent a smaller size. Only one percent expressed dissatisfaction with their scars, which were usually located in the crease under the breast. Seventy-five percent of women rated their breast firmness “just right.” When asked to rate the result, over half of the patients gave it a perfect 10 on a scale of 1 to 10. Almost all patients (98 percent) reported that the results met or exceeded their expectations.
Increased Self-Esteem and Other Psychological Benefits
Nearly 40 percent of the women surveyed experienced at least temporary nipple numbness after surgery. Persistent numbness was reported by only two percent. The complication rate reported by patients was 10 percent. When asked about psychological effects, 92 percent of women reported improved self-esteem after breast augmentation and 64 percent reported an improved quality of life. Before surgery, the majority of women (86 percent) were self-conscious about their breasts. After surgery, only 13 percent were self-conscious about them.
Breast augmentation is the most popular cosmetic surgery procedure in the United States – approximately 286,000 operations were performed in 2012, according to ASPS statistics. The study results provide new information for women considering breast augmentation, particularly in terms of their expectations of the recovery and results. “This study is valuable in providing information from the patient’s perspective, not the surgeon’s,” according to Dr. Swanson. “Patient satisfaction is the most important measure of surgical success.”

Augmentation mammoplasty, or breast augmentation is a surgical procedure to increase the size, shape or fullness of a woman’s breasts. The surgeon places silicone, saline or alternative composite breast implants under the chest muscles or breast tissue.
Breast augmentation is done to:

  • Enlarge breasts that are naturally small.
  • Restore breast size and shape after pregnancy, weight loss or breastfeeding.
  • Restore symmetry when the breasts are asymmetrical
  • Restore the breast(s) after surgery as treatment for breast cancer or some other condition or event that affected the size and shape of the breast.

Breast enlargement can boost self esteem- women usually experience a significant boost in self-esteem and positive feelings about their sexuality after undergoing breast enlargement.
98% of women who underwent breast augmentation surgery said the results met their expectations. Women also reported improvements in quality of life and self-esteem after their procedure.
A study, involving 225 women who were followed for five years, found that:

  • Patients said they were “back to normal” within 25 days of their surgery
  • 85% said their new breast size was “just right”
  • 13% said a larger size would have been better
  • Less than 2% would have preferred a smaller size
  • 1% were not happy with their scars, which are usually in the crease under the breast
  • 75% thought their breast firmness was “just right”
  • 98% said their results met with their expectations

What are breast implants?

A breast implant is a medical prosthesis that is placed inside the breast to augment, reconstruct or create the physical form of the breast.
The outer layer of breast implants is made of firm silicone. The inside of the implant is filled either with salt water (saline solution), silicone gel, or a composite of alternative substances. Makers of breast implants say they should last for at least ten years.

There are three main types of breast implants:

    • Saline implants – filled with a sterile saline solution (biological-concentration salt water 0.90% w/v of NaCl, ca. 300 mOsm/L.) which is held within an elastomer silicone shell. These implants can be filled with different amounts of saline solution, which may affect the feel, firmness and shape of the breast.If this type of implant leaks, the solution will be absorbed and expelled by the body naturally.

    • Silicone implants – filled with a viscous silicone gel which is held within an elastomer silicone shell.If a silicone-filled implant leaks, the gel will either stay in the shell or escape into the breast implant pocket. A leaking silicone-filled implant might not collapse. Patients choosing this type of implant should carry out more regular checks with their doctor compared to those on saline solution implants. The condition of the implants can be determined with an MRI or ultrasound scan.

What happens during the breast augmentation procedure?

Anesthesia

The patient may have a choice of general or local anesthesia. Under local anesthesia the patient is awake and only the surgical area is numbed. However, in most cases the procedure is done under general anesthesia.

The incision

The surgeon and patient should discuss incision options. The doctor should explain which are most suitable for the patient and her desired outcome.
The following incision options are possible:

  • Inframmammary incision – in the crease under the breast.
  • Transaxillary incision – in the armpit (axilla)
  • Periareolar incision – around the nipple

The choice of which incision to use depends on several factors, including the degree of enlargement, the patient’s anatomy, the type of implant, and surgeon-patient preference.

Inserting and placing the breast implant

The breast implant is inserted into a pocket. Today, there is a choice between two types of placements:

  • A submuscular placement – under the pectoral muscle. When placed here the patient may take slightly longer to recover, compared to subglandular placement, and may also experience a little more post-operative pain.
  • A submammary (subglandular) placement – behind the breast tissue, over the pectoral muscle.

Techniques used today for placing breast implants have significantly improved recovery times.

The incisions are closed

The surgeon closes the incisions with layered sutures (stitches) in the breast tissue, and with skin adhesives, sutures and surgical tape to close the skin and keep it closed.
Initially, there will be evident incision lines. These will fade with time.

  • clothes that fit well around your hips are often too large at the bustline

He or she will consider such factors as the size and shape of your breasts, the quality of your skin and the placement of your nipples and areolas (the pigmented skin surrounding the nipples). If your breasts are sagging, a breast lift may be recommended in conjunction with augmentation.
    You should come to the consultation prepared to discuss your medical history. This will include information about any medical conditions, drug allergies, medical treatments you have received, previous surgeries including breast biopsies, and medications that you currently take. You will be asked whether you have a family history of breast cancer and about results of any mammograms. It is important for you to provide complete information.
    If you are planning to lose a significant amount of weight, be sure to tell your plastic surgeon. He or she may recommend that you stabilize your weight prior to undergoing surgery. If you think that you may want to become pregnant in the future, you should mention this to your surgeon. Pregnancy can alter breast size in an unpredictable way and could affect the long-term results of your breast augmentation. There is no evidence that breast implants will affect pregnancy or your ability to breast-feed, but if you have questions about these matters, you should ask your plastic surgeon.

    What type of breast implants can be used for breast augmentation?

    The implant is placed in a pocket either directly behind the breast tissue (right) or underneath the pectoral muscle which is located between the breast tissue and chest wall. 

    The size and type of breast implant recommended for you will be determined by your goals for breast enhancement, your existing body frame, and mass, your existing breast tissue, and the preferences you and your plastic surgeon discuss.  All breast implants include a solid silicone rubber outer shell, called a lumen.
    Breast implant options include:

    • Saline breast implants: Filled with sterile salt water
    • Silicone filled breast implants: Filled with soft, elastic gel

    New scientific data on the safety of breast implants is rapidly being collected. In the future, it is possible that additional types of filler materials may become available as advancements take place.
