What To Expect From A Photofacial
A photofacial or IPL, is a 30-minute procedure in which intense pulses of light are used to penetrate deep into the skin. Photofacial then causes collagen and blood vessels below the epidermis to constrict, reducing redness and age lines. The procedure involves only minimal discomfort, while the redness and swelling that sometimes occur after treatment disappear shortly. Most benefits of a photofacial occur gradually in the weeks following treatment
DAY OF THE PROCEDURE
Before the procedure begins, make sure your skin is free of any tanning products or makeup. Avoid prolong sun exposure and any topical retinoids. Your skin will be cleansed and a cool gel will be applied to the treatment area, both to lubricate the handpiece and cool the skin. You will be given eye protection to protect your eyes from the bright flashes of light.
DURING THE PROCEDURE
During the IPL/photofacial procedure, you will feel a warm sensation caused by the handpiece. Some patients describe the sensation to a light “snap” from a rubber band. The number of pulses you receive will depend on the size of the area being treated. Following the procedure, your skin care specialist may help you in icing down the treatment area to help soothe any discomfort from the procedure. This is often unnecessary, however, as IPL/photofacial treatment causes very little pain.
AFTER THE PROCEDURE
The recovery time from an IPL skin procedure is often very short, as there is very little damage done to the skin during treatment. Depending on the skin condition you are seeking to remedy as well as your skin type, the redness that is present following the procedure may persist for a longer or shorter period of time. The side effects of IPL/photofacial treatment dissipate within a few hours for many patients. For some patients, however, the redness or dark spots may take a week to fade. Patients treated for severe acne sometimes experience longer recovery times, though they are often amazed by the results when the side effects wear off.

Here at Paragon Plastic Surgery and Med Spa the HydraFacial is ideal for all skin types and targets hyperpigmentation, oily/congested pores, fine lines, and wrinkles. With no down time, it leaves your skin glowing and looking healthier in about 30 minutes. For a standard HydraFacial there are four steps.
Step 1: Cleansing and exfoliation. The hand piece gently glides along the skin using a Vortex-Exfoliation combined with the Activ-4 serum that removes dead skin cells. The key ingredients are glycolic and lactic acid, algae extract that calms  and soothes and yeast extract that firms and smoothes the appearance of fine lines.
Step 2: Acid peel. A mixture of glycolic and salycilic acid delivers the benefits of a peel without the post-peel scaling. It also helps to break up dirt and clogged pores.

Step 3: Extractions. Instead of performing manual extractions, the HydraFacial uses a Vortex-Suction that acts as a vacuum to clean out pores with the help of the Beta-HD serum. This serum uses honey extract that adds a boost of hydration and anti-bacterial benefits, Spirea Ulmaria Flower extract that soothes and calms and salicylic acid gently exfoliates and aids in extractions.
Step 4: Serum application. The Antiox+ serum is now delivered to nourish and protect the skin with Antioxidants like Horse Chestnut Seed, Green Tea and Arnica Flower Extracts. TonePerfect Complex addresses skin tone by reducing melanin production and preventing the appearance of brown spots and discoloration. It also instantly firms and tightens. Hyaluronic Acid helps to plump and deeply hydrate and Earth Mineral Peptides boost collagen and elastin production to help eliminate fine lines. Sunscreen is then applied and you on your way to healthy, radiant skin.
With just one treatment you can visibly see that your skin is more hydrated and youthful.  Monthly treatments are recommended for best results. Let Dr. Mark Bishara and his staff guide you on your journey to healthy skin with the help of the HydraFacial MD.

The holiday season with all its parties, family gatherings, and New Year’s resolutions can be stressful for men and women of all ages because this is typically a special time when they want to look and feel their best despite all the stress frequently associated with holiday events.
Dr. Mark A. Bishara, M.D. P.A. and his staff at The Paragon Plastic Surgery and Medspa can help each individual man or woman look and feel their best for all holiday events coming up.  Dr. Bishara takes the time to cultivate individual relationships with his patients in order to assure that they receive their desired look.  He uses the term “patient” because he says, “I feel the term patient is a more personal and appropriate term because I am working with people in personal ways.”
In his new state-of-the art facility completed in 2014, he offers the full gamut of cosmetic procedures such as robotic hair restoration, breast augmentation, tummy tuck and a variety of laser procedures. The calming music and soothing décor of his new facility create an atmosphere to ease the nerves of any anxious patient.  “Sometimes patients come in extremely nervous, and my goal is to provide a calming, relaxing environment to reduce their anticipated anxiety,” he said.  Additionally, Dr. Bishara takes the time to listen, discuss and explain all treatment options before outlining an appropriate treatment plan.
In his Paragon office in Mansfield, Dr. Mark A. Bishara, M.D. P.A. offers “Wrinkle-Free Wednesday.”  Patients can come in for Botox and Xeomin treatments at discounted rates.  It’s easy to see why this is the most popular and busiest day of the week for Dr. Mark A. Bishara, M.D. P.A. and his staff.  He treats hundreds of patients seeking these non-invasive, preventive procedures from 8am until 8pm.  “We go through enough Botox to remove every single wrinkle from an elephant’s skin,” he grinned, “but my goal is for them to still look like themselves,” he explained.
Also, as an AAAHC certified facility and dual trained surgeon, he can provide acute care as well as cosmetic surgical services.  His goal is to make sure that every patient’s needs are satisfactorily and effectively met.  In the spirit of giving this holiday season, Dr. Mark A. Bishara, M.D. P.A. plans to open a combined clinic that will offer reduced rates for general surgical procedures including lumpectomy, advanced laparoscopic hernia and gallbladder procedures, and varicose vein ablation.
“The current medical system in this country is not sustainable.  Hospitals and surgery centers charging $11,000 for a 15-minute procedure is simply not feasible when it could be offered at a more reasonable price,” he said.  These large centers are sometimes bound to hold pricing high due their contracted rates with insurance companies.  As a responsible physician, he wants to provide clinical services that promote and foster a medical system that is sustainable and affordable.  That is why he has announced the addition of four advanced laparoscopic surgeons to the staff at the Paragon Surgery Center in Mansfield.  Surgical procedures are now being offered to all patients state-wide for a cash price that is often less than some deductible plans.  Although this system is intended to be cash based, some patients will still be able to submit the charges for reimbursement through their out of network benefits.
As his services continue to grow and expand, Dr. Mark A. Bishara, M.D. P.A. has two other locations for the convenience of his patients; Southlake and North Park.
As the holiday season and the new year approach, let Dr. Bishara and his staff help you look and feel your best.  Call one of his three convenient locations at 817-473-2120 or email appointments@markbisharamd.com to make an appointment and ensure that you enjoy the holidays knowing that you truly look your best!

From topical treatments that lighten to in-office lasers that ablate the upper lay of skin, there are a myriad of options to limit the look of the sun-damaged skin.  Topicals are almost always the first recommendation to address sun damage, and are often coupled with more aggressive treatments.
Topical Treatments:
Start with retinoids- and retinol, which are derivatives of vitamin A.  They work to thicken collagen, regulate cell turnover and lighten spots.  Add in serums with growth factors and peptides to strengthen skin that has been weakened by the sun. Lighten up with skin brighteners that diminish pigmentation in the skin.  Hyrdoquinone is the most effective bleaching ingredient, but licorice soy and kojic acid help, too.
If you have freckles and minimal spots: IPL (Intense Pulsed Light)
Although the least intense type of sun damage- they can be dark and large in quantity. Freckles tend to surface on the skin as early as childhood.  IPL treatments are the best tool to get rid of both red and brown pigmentation.  It does a great job of normalizing color, so you can really get those light and dark brown spots.  The more you do this, the more collagen that forms, so your skin looks healthier and smoother.  IPL is a great treatment for someone who isn’t ready for a factional laser yet.
If you have severe age spots: Hydroquinone, Kojic Acid, Retin-A, and Laser Resurfacing
Ranging in color from brown to almost black and in size from something bigger than a freckle to as big as a coin, age spots are a direct result of excessive sun exposure that has prompted an overproduction of melanin.  When it’s an isolated brown spot that isn’t too dark, hydroquinone or kojic acid, used with Retin-A, can help reduce discoloration.  At The Paragon Plastic Surgery & Med Spa, we have our Paragon Cream that offers all 3 ingredients. More aggressive procedures are offered, like lasers that really work to resurface the skin.  The principle is that the body recognizes a wound (created by the laser) and heals it with the new collagen and new skin.
If you have skin that is uneven in texture: Exfoliation and Chemical Peels
The sun sucks the life right out of your skin.  After skin is damaged, it looks older than it should and more leathery in texture.  It’s not smooth or youthful looking because there is a lack of healthy collagen and elastin, making the skin look rough, dull and weathered.  Regular exfoliation with salicylic, lactic and glycolic acids can help.  You’ll see the most improvement when they are used in tandem with a prescription strength retinoid to further smooth the skin.  Chemical peels physically exfoliate the top layer of skin and repair some of the damage with specific ingredients, namely acids.
If you have red broken blood vessels: IPL Photofacials
Small and thin, blood vessels can easily break from anything from stress, pregnancy, being too aggressive on skin and even the sun.  But the problem comes when skin tries to repair them and can’t.  Cumulative sun damage prevents skin from regenerating these damaged vessels properly and so the redness lingers.  Broken vessels respond well to IPL.  The laser works to take the redness out because the wavelength of the machine targets the hemoglobin in blood, which is what gives the skin the redness.  The vessel almost bursts or is reabsorbed by the body over time and the broken capillary goes away.
