The holiday season with all its parties, family gatherings, and New Year’s resolutions can be stressful for men and women of all ages because this is typically a special time when they want to look and feel their best despite all the stress frequently associated with holiday events.
Dr. Mark A. Bishara, M.D. P.A. and his staff at The Paragon Plastic Surgery and Medspa can help each individual man or woman look and feel their best for all holiday events coming up.  Dr. Bishara takes the time to cultivate individual relationships with his patients in order to assure that they receive their desired look.  He uses the term “patient” because he says, “I feel the term patient is a more personal and appropriate term because I am working with people in personal ways.”

“The current medical system in this country is not sustainable.  Hospitals and surgery centers charging $11,000 for a 15-minute procedure is simply not feasible when it could be offered at a more reasonable price,” he said.  These large centers are sometimes bound to hold pricing high due their contracted rates with insurance companies.  As a responsible physician, he wants to provide clinical services that promote and foster a medical system that is sustainable and affordable.  That is why he has announced the addition of four advanced laparoscopic surgeons to the staff at the Paragon Surgery Center in Mansfield.  Surgical procedures are now being offered to all patients state-wide for a cash price that is often less than some deductible plans.  Although this system is intended to be cash based, some patients will still be able to submit the charges for reimbursement through their out of network benefits.

There you are, living your life and so proud that you’ve gotten into the habit of using daily sunscreen, when one day you look down and, What? Are those sun spots on my feet?
Sorry, but yeah, probably. It’s so easy to consistently miss spots either accidentally or because you don’t think they get enough sun exposure to warrant sunscreen application. And since sun damage is cumulative (five minutes here, 10 there, over the course of a lifetime really adds up), it’s super important to take extra care to cover neglected skin as to keep your entire face and body looking young and fresh for as long as possible.
We’ve zeroed in on three body parts where sun damage commonly sneaks up. If you already see damage there, you can take steps to undo it, and if you don’t, then there’s no time like the present to take a proactive approach.
Spotty Chest
Why: V-necks, sheer fabric and itty bitty bikinis leave the vulnerable skin on your chest exposed to regular UV assault. Even sporadic exposure from the car or walking in and out of buildings contributes to this damage and discoloration.
What to do: Targeted products with brightening ingredients such as kojic acid, licorice extract, soy or niacinamide can help. Follow your normal skincare routine, but bring it down to the chest, and daily sunscreen is key to keep the spots from getting darker. For even more prevention, cover up with a scarf while dining outside or at sporting events.
Discolored Hairline
Why: So, you have a mysterious brown headband up at the top of your forehead? This is a common issue because the top of the forehead gets a lot of sun exposure, but people don’t like putting sunscreen there because it makes the hair greasy.
What to do: The best way to prevent this is with a hat, but at the very least, be sure to blend your sunscreen all the way up to your hairline. Matte or cream-to-powder formulas won’t gunk up your hair. To fade discoloration, the same ingredients that work on your chest (see above) work here, and additionally, chemical peels and IPL treatments in a derm’s office can help. Severe cases could benefit from a prescription retinoid or hydroquinone.
Old Feet
Why: Anyone who loves the freedom of wearing flip flops is prone to haggard feet, and the skin there is thinner than on the legs. Sun damage here can range from rough texture and fine lines to pigment changes.
What to do: Apply sunscreen and let it dry before slipping into sandals. Sunscreen formulated for active lifestyles and sports stays put longer, and handy sprays make it easy to coat your legs and feet. At night, wash and scrub your feet thoroughly and apply foot cream to keep skin soft and young.

As summer is soon approaching, and we are out in the sun more and more, we want to educate patients on sun-safe behaviors and the selection and use of sunscreen for maximum protection.
 
Here are three important points to remember when shopping for and using sunscreen this summer.
1. Make sure your sunscreen is labeled “BROAD SPECTRUM” meaning it protects against both ultraviolet A (UVA) and ultraviolet B (UVB) rays. Both UVA and UVB radiation contribute to the sun’s damaging effects, which include sunburn, skin cancer, and premature skin aging.  The best sunscreen offers protection from ALL UV light.
2. Choose an SPF or 15 or higher — only broad spectrum sunscreens with an SPF of 15 or higher can state that they reduce the risk of skin cancer and early skin aging (when used as directed).
3. Use sunscreen early and often. Apply a good sunscreen liberally, and reapply after swimming or excessive sweating. There is no such thing as “waterproof” sunscreen. That term is no longer allowed on labels. Any claims of water resistance must indicate whether the sunscreen remains effective for 40 minutes or 80 minutes while swimming or sweating, based on standard testing.
But wait, there’s more! While knowing how to select and use the proper sunscreen is an important part of protecting your skin from the sun, these additional sun safety measures should be part of your routine this summer.

  • Avoid sun burns, intentional tanning, and use of tanning beds
  • Wear sun-protective clothing, a wide-brimmed hat and UV-blocking sunglasses
  • Seek the shade, especially between 10 AM and 4 PM
  • Use extra caution near water, snow and sand
  • Get vitamin D safely through food and supplements.

Enjoy your summer, but don’t let it damage or prematurely age your beautiful skin!

While some learn to love those tiny brown spots on the tip of the nose, cheeks and shoulders, others look for anything to fade freckled skin. Thankfully, you can lighten unwanted freckles and spots instead of just covering them up with makeup. And, obtaining brighter, smoother, spot-free skin may be easier than you think.  At the office of Dr. Mark Bishara we want to educate our patients about proper skin care and help them improve the health and appearance of their skin.
Protect and Prevent
With a medical name like ephelides, it’s no surprise that freckles get a bad rap. While “simple freckles,” the tiny brown spots on your skin you have had since birth, are harmless, lentigines, or “sunburn freckles,” need to be taken more seriously. Usually larger in size, more red in color and with an irregular border, these spots are sure signs of sun damage. No matter how old you were when you first noticed your freckles, sun exposure can only make them darker and greater in number. The easiest way to prevent freckles from forming is by using sunscreen. “I recommend applying sunscreen with at least an SPF of 30,” says New York and Miami dermatologist Fredric S. Brandt, MD. “No product will effectively lighten skin without the use of sunscreen on a daily basis.”

On the Bright Side
Over-the-counter brighteners can help to even out freckles and spots for more evenly toned skin. Zap Away
For serious long-term freckle fading, laser resurfacing lightens and retexturizes. Unlike chemical peels and microdermabrasion, which just exfoliate, pigment-specific lasers, like the Alma laser systems, specifically destroy pigment in the epidermis and dermis to eliminate discoloration with minimal side effects. Fractional resurfacing lasers poke tiny holes into the skin to deliver a dose of controlled heat to the deeper layers, eliminating freckles from the inside out. Not a laser, but similar in terms of results, Intense Pulsed Light (IPL) uses broadband light to diminish freckles. Not all lasers are alike, so it’s important to diagnose your skin accurately prior to treatment.
